Summer Application
Summer @ Oakwood 2021
This Application Form is for students not currently enrolled at Oakwood School, but who are applying to participate in Summer @ Oakwood 2021. After completing your online Application, please email your supporting documents to summer@oakwoodschool.com. When the Application process is satisfactorily completed and your child is officially accepted into the summer program, the Summer Team will contact you if an appropriate summer spot opens up for placement. Please complete the Application as early as possible.
To complete the Application process, please submit the following items:
- Completed online Application for Summer 2021
- Most recent Psycho-educational evaluation and report
- Speech/Language evaluation
- Occupational Therapy evaluation
- Current Individualized Education Program (IEP) or 504 Plan
- Information on any medical conditions
- Most recent school Report Card
- Other relevant tests and reports
- ($55 Application Fee - Do not send payment at this time)
