Summer 2026 Back-to-School Checklist

• T-Shirt Order Form - due Friday, July 31st
  • Submit your child's t-shirt size using the form below to order an Oakwood t-shirt for the upcoming school year.
• Medical Forms - due Friday, August 21st
  • New students: submit a Virginia Health Form signed by a health care professional within the past year (email or use the form below).
  • Rising 7th-graders (new & returning): submit an updated immunization record. 
  • Students with a documented allergy or medical condition requiring medication or an action plan: complete new forms before the school year begins (blank forms below).
• Summer Reading Project - due Wednesday, September 2nd
  • For returning students from teams 5-11 and new students entering grades 6 or 7:
    • Review the project description. 
    • Download one of the paragraph planners and complete it by hand or type your answers.
  • Homeroom teachers will expect to receive the following items by the deadline:
    • A completed paragraph planner, typed or handwritten.
    • A rough draft of the summer paragraph with marked edits, typed or handwritten. 
    • A final copy of the summer paragraph, typed. 
    • A completed project from the list of options listed in the description.
